2. Appointment Requests Are Not Guaranteed

Submitting an appointment request through this website or our patient app does not confirm an appointment until our office contacts you or the patient app displays a confirmation. For urgent dental needs, please call us at (615) 614-2201.

This website is not intended for dental emergencies. If you are experiencing a dental emergency, call our office during business hours or visit the nearest emergency room if outside business hours.

3. Patient App Accounts

Our patient app is available to current and prospective patients of Smiles on Main. Authentication is by phone number and date of birth, verified against your patient record. You are responsible for keeping your login device secure.

If you believe your account has been accessed without your permission, contact our office immediately.

• Use the Services for any unlawful purpose.
• Attempt to gain unauthorized access to our systems or other patients' records.
